Improve protocol and type schema generation.

The code doesn't rely on `JSON.stringify` anymore.

Breaking changes (not including `getSchema` now returning an object,
previously committed):

+ `type.toJSON` is now aware of de-referenced types and will correctly
  handle multiple occurrences of the same type. This enables the use of
  `JSON.stringify(type)` to generate a type's canonical schema.
+ Removed `Field.inspect`, `Field.toJSON`, `Message.inspect`,
  `Message.toJSON`, and `Message.toString`.
