Refactor url building (#199)

* Move helpers into "-private" import path

By doing so, we mark them as part of the internal workings of the
library. If their API needs to the change in the future, by the
conventions of the way that Ember addons usually work, we don't need to
release a major version.

* Refactor `parse-response-headers` and its tests

* Refactor away the RequestURL class

* Refactor URL building code

Makes the code a bit cleaner, while still satisfying the tests
