Start the big refactoring

- Started to split autoscaling.go in more manageable pieces.
- Slightly cleaner instance handling, more to come.
- Removed raw data from the AutoScaling group and Region data
  structures, now they just extend structs defined in the SDK.
- Introduced a debug log level based on an environment variable.
- Fixed a few more logging issues.
- Terminate spot instances for which there's no on-demand instance
  anymore which could be replaced.
- Some more variable renames and random cleanups.
