apply various refinements

* use `Buffer.from()` instead of the deprecated `Buffer` constructor
* fix `app.inspect()` in REPL (by fixing `app.root`)
* set default value of `ctx.type` to `text/html; charset=utf-8`
* ignore falsy `res.etag` values
* remove development assertions of `Promise` and `Generator` bodies
* send text bodies always with utf8 encoding
* don’t remove content headers in response to HEAD requests
* improve ‘content-type’ generation logic
* introduce `res.charset()` to manipulate ‘content-type’ header easily
* change affected unit tests to fit to the changes mentioned above
* port `res.send()` sends from Express
