Case-insensitive header matching (#187)

* added get-header util function + tests

* updated isJSONAPIContentType to use case-insenstive regex match

* changed Content-Type header checks to use case-insensitive getHeader function

* made 'text/html' content-type check case-insensitive

* removed vestigial jshint directive

* changed 'let' to 'const' for non-changing variables in get-header & tests

* changed get-header tests to use e'expect' rather than 'assert'
