Use service WithConfiguration to not rely on global setup (#13)

* Use service WithConfiguration to not rely on global setup

This avoids mucking with global providers and makes testing
easier/cleaner/more obvious.

* Remove WithConfig global test helpers

Again trying to move away from modifying global state in tests.

Use the WithConfiguration() option to set a configuration provider.

* Address review comments
