[#784] Entry created by entry form submission.

New Entry instance is created in db when entry form is submitted, as
long as no current entry already exists. Code largely similar to the
previous version of the POST handler.

Question answers are now saved as one JSON blob, rather than individual form
fields. Which means we don't have to specifically strip out the other
additional form fields when preparing the object to save from the
submitted form data.
