Fix error with absolute paths

stylelint will pass the relative path to process.cwd() into code(), but
the absolute path to the file into result() – meaning it worked fine in
the unit tests, but the source map correction failed for real projects
because we assigned a relative path in code(), but relied on an absolute
path in result().
