feat: record broadcast request outcomes over HTTP (#2428)

Record one outcome for each Broadcast message that a client sends over
HTTP. This covers the single endpoint and the batch endpoint. The outcome
uses the same event and metric as the WebSocket path.

SingleBroadcast.broadcast/7 and BatchBroadcast.broadcast/4 return their
outcome next to their result. The controllers call RequestMetrics.track/4,
which records each outcome and returns the result unchanged. A crash is
recorded as an internal failure and raised again.

The controller records the requests that it rejects before it calls
SingleBroadcast: a body that is not a JSON object, and a persist option that
is not valid. The error text and the status code do not change.

A batch gets one outcome for each message. An invalid batch still fails with
a 422, but each message keeps its own reason. A list with an entry that is
not an object counts one invalid request for each entry. The batch handlers
for authorization errors use the same error mapping as SingleBroadcast, so
a database failure counts as an upstream failure.
