feat: read [tool.holoscan] at standalone application roots

Standalone applications now read the same [tool.holoscan] keys as other
roots without a Module, so every project can set its defaults in its
root pyproject.toml. The configuration guide now states that rule
without describing each kind of root.

When HOLOSCAN_CLI_REPO_PREFIX overrides the repository prefix and no
container-prefix is configured, image names now follow that prefix
instead of one derived from the project's setting, so images and the
workspace folder use the same name.
