fix(validate): run the file rules on a struct or map source

fileValidate required the data source to be a *FormData:

	form, ok := v.data.(*FormData)
	if !ok {
		return statusFail
	}

so isFile/isImage/inMimeTypes failed for every struct or map field, no
matter what the field held: the same upload passed through
validate.Request(r) and failed through validate.New(&form{File: fh}) with
"File value must be an image".

A non-FormData source now looks the field value up and, when it is a
*multipart.FileHeader (or a slice of them), wraps it in a temporary
FormData, so the three rules keep a single code path. A field that is not
an uploaded file keeps failing as before, and an empty field follows
skipEmpty. MIME types are still sniffed from the file content.

The READMEs now say the rules accept a *multipart.FileHeader field too.
