Skip hidden Pile rows and Columns columns in focus moves

A row or column the layout draws 0 high or 0 wide, and that focusing
would not show, is now treated the same in every sizing mode.

- Initial focus skips such children unless no other child is selectable:
  Pile rows of height or weight 0, Columns columns of width 0 and of
  weight 0 when min_width is 0.
- Pile records the rows the last layout hid: weight 0 outside a flow
  Pile, and flow or fixed rows that are 0 rows focused too. Up/down,
  Tab/Shift+Tab and focus_first/last_selectable skip them.
- Columns also records weighted columns its weights and min_width left
  0 wide, and both hidden sets in a fixed layout.
- keypress lays out again after the focus child handled the key, since
  the child may have changed the contents, which clears the sets.
- A weighted row left 0 rows for lack of room stays reachable: focusing
  it gives it rows.
