mapper: don't panic on nil converters

Mapper kept the default converter as a pointer to an interface, so
WithDefaultConverter(nil) stored a non-nil pointer to a nil interface,
and Map panicked on the first field beyond the converters list. A nil
converter in the list made Map panic the same way.

The default converter is kept as an interface now, and a nil one
means there is no default converter, as if it was never set. A nil
converter in the list makes Map return an error.
