Migrate to uv (#14)

Switch to uv as package manager frontend (hatchling/hatch-vcs backend) per
hdx-scraper-copier template. Consolidate hatch, ruff, pytest and coverage
config into pyproject.toml and replace requirements*.txt with uv.lock.
Update workflows (renamed to .yaml), pre-commit, .gitignore and README, and
add CLAUDE.md and docs/decisions. Apply ruff 0.17.3 lint fixes.
