docs: publish to GitHub Pages instead of Read the Docs

Read the Docs built against cijoe from PyPI, so the documentation described
the last release rather than the tree it was built from, and nothing caught
the packages section going stale. A docs workflow now builds from the
checkout on every push and pull request, fails when docs/source/packages
differs from what emit.py produces, and deploys to Pages from main.

Read the Docs keeps serving cijoe.readthedocs.io, but only a page that
forwards each address to the same page on https://refenv.github.io/cijoe/,
used as its 404 page too so deep links survive. The README, the project
URLs and the report template move to the new address; the template loaded
its logo from Read the Docs, which would otherwise have broken.
