Address review comment r4402619644. Keep 20,000 total scenarios while exercising all four scaling-policy combinations.

Address review comment r4402619685. Propagate the selected scale-down policy during overlap and retain the existing aggregate scenario budgets.

* test(disaggregatedset): reopen scale-up after observed old reduction

Address review comment r4402619709. Keep desired at 14 while old Spec drops from four to two, requiring Immediate to grow the target from ten to twelve for both group identities and scale-down policies.

* test(disaggregatedset): keep External clamp independent of scale-up

Address review comment r4402619727. Immediate scale-up retains the in-flight External target with both omitted and explicit Deferred scale-down.

* test(disaggregatedset): cover rollout scale-up policies end to end

Address review comment r4402619737 with Ordinal and Hash E2E cases covering all four independent scaling policy combinations. Verify held readiness, request-based scale-up, readiness loss, scale-down, and final convergence. Update the fixtures, user guide, and KEP history in this PR rather than deferring them.
