Restructure the documentation for new users (#320)

Group the sidebar into Getting started, Guides and Reference, and add a
Quick start that the home page now links to.

- Add Quick start, CI integration and CLI reference pages
- Move risk gates and exceptions out of the configuration page
- Move the list of risk signals out of the risk reports page
- Expand the usage page into a full guide to running a scan
- Remove duplicate sections from the risk and caching pages
- Fix errors: allow-vs-deny precedence, explain and the cache,
  the --skip-restore flag name, a missing denyKnownExploited example
  and a shipped roadmap item
