Reduce mock usage in tests, refactor helpers (#3528)

### Summary

Round of revising usage of `unittest.patch` and related functionality in
`test/unit` test, as well as revisiting some helpers:
* remove `test/unit/mock`, conceptually a "folder for constructs needed
for tests"
* replace the `FakeMidcircuit` backend with modifications to
`FakeManila` in `alternative_backend_snapshots()`, in the same spirit as
recent tests over new gates
* replace `FakeCircuitScheduleInputData` with finer-grained fixtures in
the visualization module, instead of a data-holder-ish class
* replace tests that used faulty devices (with success) and asserted
over the inner `_run` methods being called with `check_faulty`
* other targeted changes

This is not meant to be exhaustive: most of the remaining direct usages
are in the executor-related tests.
