* docs(plugin): correct the clean-exit contract in the diff-review skills

the terminal step claimed "the review is complete" when revdiff can only
observe that no annotations were captured. It never sees whether the
reviewer approved, so the wording asserted more than the tool knows and
read as authorization to proceed.

both diff-review copies now say no annotations were captured, tell the
agent to inform the user and stop the review workflow, and state that the
result alone authorizes no further changes while prior explicit user
instructions still apply. Covers How It Works step 6, the Step 6 terminal
branch, Step 7, and both example endings.

the pi copy already worded this correctly and is untouched, as is the
plan-review skill.

* docs(plugin): correct the clean-exit contract in the codex plan skill and pi result

the codex revdiff-plan skill and the pi tool result still called a quit
without annotations a completed review. Both now say no annotations were
captured, and the plan skill states the result alone does not approve the
plan or authorize implementation.
