one-shots with an IB cancel cancel at TWS on drop (#1107) (#1109)

head_timestamp, histogram_data, historical schedule, option price, implied
volatility, wsh metadata and wsh event data read through a plain internal
subscription with no cancel: a dropped future, timeout or local error left
the request running at TWS against pacing.

- request_helpers::one_shot_cancelling::<K> reads a typed
  Subscription<OneShot<K>>; K: CancellableOneShot names payload, decoder and
  cancel. Answer and TWS error end the request; anything else cancels.
  Live: each answers with exactly one frame
- next_frame shared (historical_data reuses it)
- TickOptionComputation gets a ProtoPayload impl
- frames of an undeclared type are skipped, not UnexpectedResponse
- StreamDecoder for OptionComputation and WshMetadata removed (no consumer);
  their tests drive OneShot<K>
- one-shot-narrowing rule doc, stale notes, CHANGELOG
