[FIXED] Keep TLS server name for IPs discovered via an IP entry (#2217)

Servers learned from INFO connect_urls that are bare IPs get the current
URL's hostname as tlsName so the TLS handshake verifies against it
instead of the IP. This was skipped whenever the current URL was itself
an IP. After a failover to a discovered IP entry, a server that came
back on a new IP was added without a tlsName, and the next reconnect to
it failed certificate verification against the IP.

Inherit the tlsName of the current IP entry when it has one.
