* Forward passthrough headers only from the request

A request that omitted an allowlisted passthrough header could reach
the backend with the value sent by the request that created the
session. CaptureMiddleware stored nothing when no allowlisted header
was present, so the handler context fell back to the values captured
at session creation.

Always attach the captured map when an allowlist is configured, even
when it is empty, so an absent header is not forwarded. This matches
the documented contract that a header absent on a request is not
forwarded.

Update the PassthroughHeaders field docs, which still said the headers
are consumed once at session creation, and regenerate the CRDs and CRD
API docs. Background capability resyncs have no triggering request and
keep using the values captured when the session was created.

* Correct passthrough header docs and widen test

The PassthroughHeaders docs claimed every backend call carries the
triggering request's headers. That holds for tool calls, resource reads
and prompt gets, but session-scoped work (the session's backend
connections, including teardown, its advertised capability list, and
background resyncs) still uses the session creator's values. Say so,
and fix two server.go comments that still described capture as
feeding only the per-session client at session creation.

Extend the regression test to cover resources/read and an allowlist of
two headers where a request sends only one, bound each request with a
client timeout, and stop calling require from the backend handler
goroutine.
