Fix: fail closed when sensitive data would be skipped or leaked.

Scan rejects unhandled tags, cycles, and bad subjects instead of ignoring them, and Replace writes only after every callback succeeds. Mask errors redact the field, default redaction no longer keeps the original length, and examples no longer depend on litter.
