* docs: note that macOS keychain claude logins do not persist refreshes

The Docker wrapper bind-mounts ~/.claude/.credentials.json and ~/.codex/auth.json so token
refreshes reach the host. A Claude login kept only in the macOS keychain is extracted, mounted
read-only and copied into the container instead, so a refresh during a run stays there and can
force a new login on the host. The README said refreshes always persist.

Document the exception and the claude setup-token plus -E CLAUDE_CODE_OAUTH_TOKEN workaround,
including that the wrapper still needs a regular host login.
