Report every absence reason of a parameter, not just the first one (#1152)

* Report every absence reason of a parameter

Input-signature inference recorded one absence reason per blocking
parameter, the first its dispatch met, so a parameter blocked on several
counts at once (a call passing `None` beside calls disagreeing about the
dtype, say) reported only one of them. Report every reason that holds,
as every failed precondition of a function is reported.

The per-parameter record is now an ordered set. The flat dtype bottom
reports every axis that holds (None, dtype, sparseness), and a container
reduces every element position, each that bottoms contributing all of
its reasons. A check whose premise an earlier block denies is still not
reached, and each such site says why. The first reason of each parameter
is the one reported before, so the function-level absence reason, and
every signature, transformation and precondition outcome, is unchanged.

The evaluator writes one row per (parameter, reason) in
blocked_parameters.csv and signature_absences.csv.

* Test that every element position reports its reasons

A container parameter now reduces every element position, each one that
bottoms contributing all of its reasons, rather than stopping at the
first. Pin it with a three-position list: one position mixes layouts,
one mixes dtypes, and one does both.

A container parameter whose lengths disagree reported only that, since
the extraction stopped at the disagreement and the dispatch read its
two container findings as one if-else. They are separate findings: a
list nesting another list, or holding a Python number, beside a list of
