Make integration bundle setup automatic

Install each isolated integration bundle explicitly before running its specs, pin the runner to the CI Bundler version, and document that contributors no longer need per-integration setup. Retain external Gemfiles and fixture directories to preserve dependency isolation and fixture behavior.
