codegen: collapse per-field fieldContext_* constructors into a descriptor table

The executor emitted one fieldContext_<Object>_<field> function per schema
field, whose bodies differed only in the object name, IsMethod, IsResolver,
the child dispatcher and the args function. On large schemas these
constructors dominate generated code size and compile time.

Replace them with a fieldContextID constant per field, a fieldContextDescs
table holding the request-independent part of each FieldContext, and a
single fieldContextByID constructor. childFields_* dispatchers are now
generated once per return type, including scalars and enums, so every row
has a child dispatcher.

The table is filled in init() because a declaration initializer would form
an initialization cycle through childFields_* and fieldContextByID.
Constants and rows are grouped per object so gofmt aligns each group
independently. Both UseFunctionSyntaxForExecutionContext modes are
supported.

Field.FieldContextFunc and Field.ChildFieldContextFunc are removed since
the functions they name are no longer generated. UniqueChildFieldTypes
keeps its behaviour and is deprecated in favour of ChildFieldTypes.
