Skip suggestions when collecting project vocabulary (#7)

Use suggestion-free rule execution for collection and collect only markdownlint-filtered diagnostics so suppressed words stay excluded. Profiling 40 Markdown files with repeated typos reduced median collection time from 789 ms to 296 ms with identical candidate output. Batching offered no improvement and was not retained.
