docs: correct statements the code does not match, from the review of #158 (#159)

* docs: correct the help for the normal sample and the repeat indel filters

* docs: correct the README on filters, normalization, temporary files and the VarDictJava fork

* docs: describe folded records and the tumor sample in the header lines

ALT_READ_POS_VARIES on a folded record is 1 only when a folded row's is or the rows' distances differ, so it no longer says the flag follows REALIGNED_FRAC_OF_DP there; the per-field rounding notes say a folded record's weighted means and REALIGNED_FRAC_OF_DP are not rounded; and the PASS line says the FILTER labels test the tumor (or only) sample and that a call without ALT reads there keeps no FILTER, as the README does. Fixtures carry the new header lines.

* docs: say a partial of an MNV an earlier region wrote is dropped

The region doc now states the drop the README describes, and the integration test case that checks it is named for what it checks.
