Slim the test suite to the checks that catch real breakage (#148)

* Stop scanning tests/ for doctests; drop __doc__ assert

Move --doctest-modules out of pytest.ini's addopts so the default run
no longer scans tests/, and pass it explicitly where the loqs/ doctests
run. Remove the suite's only assertion on docstring content.

- pytest.ini: drop --doctest-modules from addopts
- CI "Docstring tests" step and devguide: `pytest --doctest-modules loqs`
- test_preserves_function_identity no longer asserts __doc__

* Slim codepack tests to fast plumbing checks

Cut the default codepack run from 330 s to 53 s on 4 workers by keeping
only checks that each instruction builds, composes and runs; codepack
correctness and fault-tolerance testing move to #66 or a follow-up.

- Cut the fault-injection sweeps and the truth-table matrices across
  layouts, modes and states; thin the rest to one small layout per
  instruction, and the 5_1_3/7_1_3 backend grid to one case per
  supported combination
- Replace the surgery CNOT tests with fast two-patch component tests:
  correction table, repair fire path, ZZ-then-XX merge pair
- Add single-fault smokes for the fault-only branches of the 5_1_3,
  7_1_3 and tomita codepacks
- Add small generic tests for the backend agreement, error injection,
  lookup decoder and history collection the cut tests exercised
- Run multi-patch STIM tests at 10 shots without random-branch asserts,
  and merge the M_ZZ Bell-prep tests

* Slim backend tests into shared state and circuit suites

Cut tests/backends from 9,177 to 4,984 lines and its run from 74 s to
25 s on 4 workers by testing behaviour the backends share once, and by
dropping wording, duplicate and minor-path tests.

- Add shared parametrized suites for the NumPy, QSim and STIM states
  (test_states.py) and the List, PyGSTi and STIM circuits
