Read a path parameter's type from the handler's signature (#9)

## Overview

A route parameter whose constraint implies no type was always documented
as a `string`, even where the handler's signature says `int $id`. The
parameter now takes its type from the handler's parameter of the same
name, when that type is a scalar.

## Summary

- A path parameter typed `int`, `float`, `string` or `bool` in the
handler's signature gets that type in the document, in both adapters
- A constraint that admits only an integer, such as `[0-9]+`, still
makes the parameter an `integer`, whatever the signature says
- A class type, such as a model Laravel binds from the route's value, is
not read, and the parameter stays a `string`
- An enum type is not read yet, because swagger-php gives an enum
without a schema of its own no type at all
- The README describes the order: constraint first, then the signature,
then `string`
- A `CONTEXT.md` records the package's terminology
