fix: publish index.js and expose the '.' entry

The 0.3.0 npm tarball ships no index.js (files omitted it) and the
exports map lacks '.', so both node and bundlers fail to resolve
'@webreflection/bindings' (vite: 'Missing "." specifier'). The repo
has index.js — include it in files and map '.' to it.
