feat(template): use the SDK helper for Module Python metadata

Generated Modules with Python bindings now call the Holoscan SDK's
holoscan_install_python_module_metadata(). The helper writes the
holoscan-<slug>-cu13 dist-info with a holoscan.modules entry point in
the build tree and the install, so import holoscan.<slug> resolves to
holoscan_<slug> without a wheel.

The Python package moves to the helper's holoscan-<slug>-python_libs
component, without EXCLUDE_FROM_ALL, so one component install carries
both the package and its metadata. The Debian package is unaffected:
packaging turns the bindings off.

The template's pytest suite checks for exactly one entry point and that
the alias resolves to the same proxy class. Requires a Holoscan SDK
5.0 build that provides the helper.
