Use module_function in OmniAuth::Utils again

Utils switched to extend self only because mutant couldn't reach
methods copied by module_function. Mutant now mutates those methods
where callers use them (https://github.com/mbj/mutant/pull/1679), so
go back to module_function, which keeps the instance methods private
when Utils is included, and drop the RuboCop override.

The fix isn't released yet, so the Gemfile takes mutant and
mutant-rspec from the commit that merged it. Switch back to a
released version once it's out.
