Keep form params from the request phase in omniauth.params

Since 2.0 the request phase is a POST, but omniauth.params only held
the query string, so params an app sent in the form were lost by the
callback phase. Store the form params too.

https://github.com/omniauth/omniauth/pull/867 stopped storing POST
params so the authenticity token wouldn't end up in omniauth.params.
Leave it out explicitly instead.
