fix(core): report policy file errors without a traceback (#426)

A policy file that exists but cannot be used stops the server (#285, and the
shape checks from #409). The RuntimeError reached the user as an unhandled
exception, so the useful message (the file, the key or the YAML position) came
after one or two full tracebacks.

Every place that rejects the policy file now raises PolicyFileError, a
RuntimeError subclass, so existing callers and tests keep working. serve()
catches it at its first read of the policy, the persona check from #411, and
exits with the message alone. The server still refuses to start.
