fix(test): add unit tests for setup_logging (#380)

setup_logging had no unit tests. Cover the json and console formats, format
auto-detection from the terminal, the log level, and that calling it twice
does not stack handlers. A fixture restores the kubeflow_mcp logger after each
test so other tests are not affected.

Fixes #379
