One server loop for connect and reconnect; rename connectReconnectOnConnect to connectReconnectOnFailure

- NatsConnection.tryServers(reconnectMode, reconnectOnInitialFailure) replaces the connect and reconnect server loops of both implementations; holdOnAuthError is derived inside it. V3 overrides stopTryingServers, invokeReconnectDelayHandler and tryToConnect.
- Nats.connectReconnectOnConnect overloads renamed connectReconnectOnFailure; the boolean is reconnectOnInitialFailure from Nats.connectAsynchronously down to tryServers. MIGRATION_GUIDE entry added.
- ReconnectTests.testReconnectOnConnect renamed testReconnectOnFailure; test enum JsSubscribeForm renamed JetStreamSubscribeForm.
- CONNECTION_IMPLEMENTATIONS.md, PLAN_NATS_CONNECTION_V3.md and the TODO Recap updated.
