molly/log: avoid formatting disabled messages

Building log message arguments happens before log.debug() checks
the level, so client.lua formatted every operation twice with
op.to_string(), which JSON-encodes its value, even when debug
logging was disabled.

Add log.is_enabled() and guard the two per-operation debug calls,
so the formatting is skipped on the default info level.
