wakatime: harden file delivery after review

A failed environment lookup no longer drops a page of file entries as
out of scope: the read position holds, the next pull retries, a lost
branch keeps the known folder, and the panel counts threads waiting on
the lookup. Pulls per thread are bounded to one running and one waiting,
and payloads at or below the read position are skipped.

The runner gains a per-job guard that re-checks settings and file types
just before spawning, so a send that waited for a slot never carries
entries the current fileActivity, entity or exclusion settings refuse,
nor a path that turned into a symlink or other non-regular entry.
Unknown fileActivity values fail closed to off, jobs are built from the
settings the send read, stopping an untracked thread resets its file
state, and extras use the cleaned project name.

Also from review: the same path problem on a different cliPath is a new
error, warnings name the thread, and a dead assignment in a server test
is gone. Docs state that paths are absolute on the server host, that
drop counts run since the plugin loaded, and every known uncounted gap.
