fix(web): a single-child hole's later content waits for a held frame like a multi slot's (#4155)

insert chose between running the inner effect's first run inline (first
render) and scheduling it into the effect phase (later renders) by
`prev !== undefined`. A multi slot's normalized value is always an array, so
that held; a single-child hole normalizes to `undefined` while it renders
nothing, so content arriving there later — a <Show> flipping to a branch
whose reads an action holds — looked like a first run, ran inline in the
compute and mounted before the commit: an enabled, empty input over a value
with no verdict (D-23). A flag set once a compute has produced its value
replaces the check.

Tests: the born-held isPending guard with the branch as the only child, for
show, show+loading and direct (show and direct fail on next; show+loading is
caught by its boundary). Web default 1390, hydrate 520, server 1724,
solid 877. Size +6 B minified.
