feat!: remove GXF extension support (#270)

The CLI no longer handles GXF extension projects:

- Delete gxf_extension.schema.json. project.schema.json drops the GXF
  language, requirements.gxf_extensions, and versioned_dependency, which
  only that requirement used.
- Discovery, list, completion, and the build-target mapping no longer
  know the gxf_extension project type, and gxf_extensions/ is no longer
  a default search path.
- HoloHubConfigHelpers.cmake drops add_holohub_extension and the
  EXTENSIONS dependency keyword. The pybind11 __init__.py template no
  longer imports holoscan.gxf; Holoscan SDK 4 already imports it from
  holoscan/__init__.py.
- validate_json() reports metadata without a supported project type
  instead of raising FileNotFoundError for a missing schema, which GXF
  extension metadata would now hit.
