feat: add integer codecs and base64url allowPadding for pvutils migration (#46)

* feat(integer): add uint and int big-endian codecs

Add the @peculiar/utils/integer entry point with unsigned (configurable
bits per byte, fixed length) and two's complement codecs that accept
number or bigint. They replace pvutils utilToBase, utilFromBase,
utilEncodeTC and utilDecodeTC without their precision limits.

Document the integer codecs and a pvutils migration table in the README.

* feat(encoding): add allowPadding option to base64url decoding

base64url.decode and base64url.is accept correctly padded input such as
"aGk=" when allowPadding is set. The default stays strict. The option is
also available through convert.decode("base64url", text, options).

* fix(integer): reject empty input in int.isMinimal

An ASN.1 INTEGER needs at least one content octet (X.690 8.3.1), and
int.decode already rejects empty input, so isMinimal now returns false
for it.

Add round-trip tests for every bitsPerByte width from 1 to 8, and note in
the pvutils migration guide that padStart does not reproduce the empty
result padNumber returned for too-long numbers.
