docs: added internals, how the library works

The classes are small and mostly clear from their signatures, so the internals keep
only what an agent or a reader would get wrong or waste time rediscovering: the three
phases of parse() with the command selection and the sentinel of a bare option, what
isset() on ParseResult answers, how a handler binds and what is checked when, and the
two questions a console answers about its stream. The how-to of add*() and the format
of the help belongs to the manual, not here.

AGENTS.md is rewritten for the same classes and points at docs/ instead of describing
the parser of 1.x.
