agent-hooks: block git hook bypass, nudge on large context

guard-bash.sh gains a soft block for skipping git hooks: the no-verify
flag and its prefixes, commit's short no-verify flag, one-shot
core.hooksPath overrides (-c, --config-env, GIT_CONFIG_* env), and
persistent git config core.hooksPath writes. It tokenizes with a fixed
python3 -I shlex program reading the command on stdin, so quoted commit
messages and heredoc bodies don't trip it; any non-verdict output falls
back to a quote-blind scan that errs toward blocking. Rules adapted from
ecc's block-no-verify.js (MIT).

New context-nudge.sh (PostToolUse) reads the last real token usage from
the session transcript and, once per 200k band up to 800k, tells the
agent to finish the step and hand off. Subagent calls (agent_id) and
Codex are skipped; state lives in .claude/logs and session-reset prunes
it after a day.
