Track store node additions and removals in edge-only views (#327)

Edge-only views contain every store node, but adding or removing a node in the store never changed their node version, so graph observers on them missed node changes and diff observers kept removed nodes. IndexStore.index also only updated the main index, so new nodes were missing from existing edge-only view column indexes. GraphViewStore now bumps the node version of edge-only views on store node add and remove, and IndexStore.index puts new nodes into the view indexes of views that contain them.
