* ci: run build and test jobs only for the paths a PR changes

Add a changes job (dorny/paths-filter) that classifies a pull request's
changed files into package, integration, and examples. The Xcode matrices,
SPM, Linux, and docs jobs run only for package changes; integration tests
also run for Tests/IntegrationTests/**; the Examples jobs also run for
Examples/**. Spell check, format check, and SwiftLint still always run.

Pushes to main and release/* and manual runs still run every job, so the
Release workflow's dependency on a successful CI run is unchanged.
ci-success already treats skipped jobs as passed, and now also depends on
changes so a failed filter cannot pass CI by skipping everything.

* ci: make the paths-filter negation exclusion-aware

With the default predicate-quantifier ("some") a negated pattern matches
every file outside it, so "!Tests/IntegrationTests/supabase*/**" made the
package filter true for any PR. Use some-with-excludes, and move the
positive rules into a shared anchor so the integration filter does not
inherit the exclusion.
