Enforce RFC 6455 frame validation in websocket handle_fragment (#1269)

* Enforce RFC 6455 frame validation in websocket handle_fragment

Fail the connection with close status 1002 when a frame carries
reserved RSV bits without a negotiated extension, when a control
frame is fragmented or its payload exceeds 125 bytes, and when
the opcode is reserved. Previously such frames were silently
ignored or answered, keeping the connection open.
