chore: add Makefile, streamline CI to use make, install Spatialite pre-test

- Add project Makefile with build/test/lint/run/docker targets
- CI uses make build / make test-ci instead of raw commands
- Install libsqlite3-mod-spatialite before tests (eliminates skip-on-CI)
- Remove Docker-in-Docker service container and testcontainers step (no
  longer needed after PostgreSQL/testcontainers removal)
