* Let forked workers drop the inherited connections without closing them

Closing them from a worker also ends them on the database side,
for the parent process that still holds them; dispose(close=False) is
SQLAlchemy's recipe for engines inherited through a fork.

register_at() now stores the status it finds on the app, and the app
fixture is shared by the whole session, so the database_schema_has_revision
